When visiting our Urologists - What should I bring?

Please bring your insurance card(s), photo i.d. and a list of your current medications with you to your first visit. We recommend that you arrive to your appointment at least 30 minutes prior to complete any patient registration and health history forms. You may print these form ahead of time from the Patient Resources sections of this website, or you may complete the forms online by selecting the "Web Clinic" button.

About Your Appointment

Clinic appointments are scheduled Monday through Friday at one of our six (6) locations. Click here for driving directions to our Fort Myers, Bonita Springs, Cape Coral, Lehigh and Punta Gorda Urology office.   Our receptionists are available until 5:00 pm daily to assist with your appointment scheduling.  You may also request an appointment online through our "Web Clinic". 

Please bring any appropriate medical documents, lab results or x-rays to your first appointment.

Our automated appointment reminder system will contact you 48 hours prior to urology appointment. If you are not able to meet with our urologist as scheduled, or are going to be late, please contact our office as soon as possible. This courtesy allows us to be of service to other patients.
